About the Role:
The Service Coordinator plays a pivotal role in connecting individuals and families with essential social assistance services to improve their quality of life. This position involves assessing client needs, developing personalized service plans, and coordinating with various community resources and agencies to ensure timely and effective support. Minimum Qualifications:
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Minimum of 2 years experience in social assistance, case management, or a related role.
- Strong knowledge of social service programs, community resources, and eligibility criteria.
- Excellent communication and interpersonal skills to effectively engage with diverse client populations.
- Proficiency in maintaining confidential client records and using case management software.
Preferred Qualifications:
- Master’s degree in Social Work or related discipline.
- Experience working within the Lakeport community or similar social service environments.
- Familiarity with local, state, and federal social assistance regulations and compliance requirements.
- Bilingual abilities to support non-English speaking clients.
Responsibilities:
- Conduct comprehensive assessments of client needs and eligibility for social assistance programs.
