What will I do as an employee with Redstone?
As a RN Triage Coordinator, supporting the Redstone mission will include the following responsibilities:
- Clear understanding of the Medicare Conditions of Participation for the hospice benefit related to eligibility, level of care, treatment and services. Knowledge and understanding of the initial and re-certification regulatory mandates, including Face to Face assessments.
- Coordinates all regulatory components of hospice patient's admission including: consents, DME, medications and facilitate changes in level of care environments as appropriate. Documents and communicates all initial admission orders to the appropriate parties to support efficient patient care.
- Expertise and utilization of all the clinical scales and assessment tools required to educate all stakeholders, internal and external, ensuring regulatory compliance. Knowledge of Advanced Directives, Living Will, and POLST.
- Knowledgeable and capable to educate and complete all legal documents associated with an admission to hospice including: Notice of Election of the Benefit, Admission Agreement/Consent for Care, and the POLST.
- Strong and effective interpersonal and problem-solving skills utilized with all stakeholders including but not limited to referral sources (referring physicians, patients, families, POA's, Hospice Medical Director, all Interdisciplinary Team members, and hospice volunteers) as well as organizational leadership.
- Triages all after-hour phone calls for client needs in all departments: Hospice, Home Health and Home Care. Recognizes the opportunity to be the after-hours hospice expert and a key consultative resource in all care environments, affording patient's access to hospice care if clinically appropriate. Conducts after-hour visits for nursing care to clients in a variety of settings. Responsible for the implementation of services to meet client needs and follows current nursing professional standards of practice and agency approved protocols for hospice. Provides ongoing assessment of clients, assuring an accurate evaluation of client needs. Identifies with clients need or quality for additional services and makes referral to appropriate IDT member. Documents the timely entry of patient evaluation/ assessment into the electronic medical record.
- Maintains and grows all professional relationships with all referral sources including but not limited to Redstone Highlands continuum of care, community physicians, physician practices, community environments of residential care and the acute care hospital representatives.
- Responsible to conduct admission assessments and OASIS for Home Health clients that require after-hour initiation of services, due to staffing needs or timeliness of acute or post-acute discharge. Participates timely in the quality review of admission documentation.
- A license in the state of Pennsylvania as a Registered Nurse (RN).
- Two years (2) of experience in Home Health, Home Care or Hospice (preferred).
