What will I do as an employee with Redstone?
As a Clinical Coordinator (RN/LPN) - Home Health, supporting the Redstone mission will include the following responsibilities:
- Provides clinical expertise and professional guidance to the department staff, while prioritizing the timely preparation of client care needs. Maintains a positive collaborative working relationship with the physician practices offices, attending physicians, and physical extenders. Supports the facilitation of the regulatory mandated care conference meetings, ensuring patient care coordination and effective problem solving.
- Responsible for the assignments of clinical staff (RN, MSW, HHA) to specific clients or care needs. Coordinates the department schedule releases, in collaboration with the Health Information Coordinator.
- Conducts initial review of new client referral processes to ensure program eligibility and appropriate physician orders. Maintains a current and working knowledge of regulatory requirements, accrediting bodies and third-party payers. Engages in effective relationship building in all referral source environments of care.
- Supports the agency operations to ensure that physician orders, assessments, plans of care and OASIS assessments are audited, transmitted and/or submitted appropriately. Works in collaboration with the Quality and Education department to assuring quality standards are met or exceeded.
- Performs joint visit as needed for specific complex complicated hospice patients for effective problem solving, customer service. Under the direction of the Clinical Team Leader, provides specific and focused coaching and mentoring of staff related to the hospice provision of care, treatment and services.
- Participates in the conducting, scheduling, etc. of in-service training and hospice education programs, on-the-job training, and orientation programs as assigned by the Clinical Team Lead. Promotes expectations so all staff understand and demonstrate excellence.
- Must be a Registered Nurse or Licensed Practical Nurse, and hold an unencumbered license to practice in Pennsylvania.
- One year of experience required in hospice, long term care or home health.
- One year experience in a Medicare-certified home health agency is preferred.
- Must possess a valid Pennsylvania driver's license and acceptable driving record.
