This pivotal role involves overseeing the development, implementation, and management of resident-centered health and wellness initiatives that promote optimal quality of life. The Wellness Director will collaborate closely with healthcare teams, residents, families, and staff to ensure personalized care plans are executed effectively. This thoughtful leader will foster a vibrant, supportive environment that emphasizes holistic well-being, safety, and compliance with all regulatory standards.
Responsibilities
- Develop and oversee individualized care plans tailored to each resident’s unique needs, preferences, and medical conditions
- Coordinate with nursing staff, physicians, therapists, and other healthcare professionals to ensure seamless delivery of services such as assisted living, nursing home care, home health, hospice care, and acute care
- Supervise and mentor wellness staff, including nurse managers and care coordinators, ensuring high standards of care and documentation review
- Manage compliance with HIPAA regulations and maintain accurate medical records using EMR (Electronic Medical Record) systems and EHR (Electronic Health Record) systems (Always ensuring compliance with Resident Rights and HIPAA policies)
- Monitor resident’s physiology and anatomy knowledge to support effective care planning and interventions
- Oversee shift management to ensure proper staffing levels for safe and efficient operations
- Conduct regular audits of medical records, documentation review processes, and care plans to uphold quality standards and Medicare requirements
- Responsible for coordinating, training, scheduling, and team development among staff
- Establishes and maintains positive, respectful, and supportive environment for all residents, staff, families, and community guests. Ensures orientation and welcoming process occurs for all new residents and new staff.
- Manages the continuing education of the staff
- Ensures all infection control guidelines are met and appropriate tracking and surveillance procedures are followed according to community and state requirements.
- Assists in planning the nursing services portion of resident discharge plans
- Leads others through consistent actions that serve our culture
Experience
- Proven management experience in assisted living facilities or nursing homes with supervisory responsibilities
- Extensive nursing administration background with nurse management experience in settings such as home health or hospice care
- Strong knowledge of medical terminology, physiology, anatomy, and healthcare regulations including HIPAA and Medicare guidelines
- Familiarity with EMR/EHR systems for documentation and medical records management
- Demonstrated leadership skills in supervising healthcare teams while fostering a collaborative environment
- Experience in shift management within fast-paced healthcare settings
- Prior involvement in developing or managing care plans tailored to diverse resident needs
Qualifications
- Graduate of an accredited nursing program
- 1 year of experience as a licensed nurse in a hospital, long-term care facility, or other related health care facility preferred.
- 1 year of Management experience
- Service-oriented industry experience (positive, enthusiastic, empathetic and respectful)
- Proven ability to handle stressful situations in a calm and professional manner
- Experience organizing tasks and managing multiple priorities

Pay: $100,000.00 - $120,000.00 per year
