Schedule: Monday to Friday, 8:00 AM to 5:00 PM, rotating on-call weekends
Position Overview
As a Physical Therapist with Redcrest Home Health, you will deliver skilled, in-home care that helps patients regain independence and improve quality of life. You will collaborate with an interdisciplinary team and play a vital role in each patient’s recovery journey.

Qualifications
- Active Physical Therapist license in Indiana
- Minimum one year of acute care experience or home health experience preferred
- New grads are welcome to apply
- Strong clinical judgment and communication skills
- Valid driver's license and active auto insurance
Key Responsibilities
- Provide in-home physical therapy services as ordered by the physician
- Evaluate patients and develop individualized plans of care
- Monitor progress and adjust treatment plans as needed
- Educate patients and families on exercises, safety, and recovery
- Complete accurate documentation in compliance with regulations
Essential Functions
- Travel to patient homes within assigned territory
- Complete scheduled visits reliably and on time
- Document evaluations and visits within required timelines
- Follow all state, federal, and agency guidelines
- Deliver care within PT scope of practice
