Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as PointClickCare.
- Administer medications via injection, oral, and other routes while adhering to aseptic techniques and HIPAA regulations to ensure patient safety and confidentiality.
- Perform specialized procedures including catheterization (including urinary), tube feeding management, ostomy care, and wound dressing changes.
- Obtain diagnostic evaluations such as laboratory specimen collection/processing.
- Provide acute pain management, chronic condition care, and participate in emergency medicine protocols that includes quick critical thinking skills.
- Educate residents and their families on health coaching topics such as geriatrics, dementia care, hospice & palliative medicine, hospice, and post-acute care planning.
- Collaborate with multidisciplinary teams to develop discharge plans, case management strategies, care plans and ensure compliance with CMS, state, and federal regulations while delivering quality care delivery.
- Maintain accurate medical documentation and ensure resident charting records are complete and up-to-date during your shift.
- Support infection control protocols by practicing proper sterile processing techniques during procedures like phlebotomy or laboratory testing, catheterization, and wound care.
- Engage in triage activities in urgent care settings in assisted living or nursing home residents. Prioritize patient needs effectively while working with individuals with disabilities, chronic illness or developmental delays.
Experience
- Proven experience as a Registered Nurse in Long Term Care, Home Plus, or Assisted Living Facilities.
- Demonstrated proficiency with EMR/EHR systems including PointClickCare.
- Skilled in performing advanced nursing procedures such as laboratory draws, catheterization, wound care, suture/staple removal, and aseptic technique adherence.
- Knowledge of infectious disease care protocols along with experience managing residents requiring dialysis or communicable diseases.
- Experience working with special populations including geriatrics (memory care, retired, etc.), behavioral health patients, or individuals with disabilities.
- Prior long term care nursing background combined with home & community care experience enhances versatility.
