Position Summary:
The Director of Social Services is responsible for leading and managing the Social Services Department and the Admission process in alignment with federal, state, and local regulations. This role ensures that residents’ mental, emotional, and psychosocial well-being are maintained and enhanced through effective assessment, care planning, counseling, discharge planning, and community resource linkage. The Director serves as a core member of the interdisciplinary care team and plays a key role in supporting residents’ quality of life and dignity within the long-term care setting.
Essential Duties & Responsibilities:
Resident Care & Advocacy
- Conduct and maintain accurate, up-to-date psychosocial assessments and progress notes for all residents.
- Identify, interpret, and address psychosocial needs through individualized care plans.
- Advocate for resident rights, dignity, and autonomy in accordance with facility policies and CMS regulations.
- Mediate concerns among residents, families, and staff to support a positive care environment.
Care Planning & Coordination:
- Actively participate in interdisciplinary team meetings, care plan conferences, and admission/discharge processes.
- Develop comprehensive discharge and transition plans in collaboration with residents, families, and external agencies.
- Coordinate resident orientation, admissions, transfers, and belongings inventory.
Regulatory Compliance & Documentation:
- Ensure all social service documentation meets regulatory, accreditation, and facility standards.
- Maintain departmental records, reports, and filing systems in compliance with confidentiality and HIPAA requirements.
- Support survey readiness and assist in regulatory inspections.
Community & Family Engagement:
- Establish and maintain effective relationships with community agencies to provide resources such as transportation, adaptive equipment, and counseling services.
- Provide counseling, education, and emotional support to residents and families regarding adjustment to long-term care and end-of-life planning.
Work Environment & Physical Demands:
- Work performed in a health care facility with potential exposure to residents who may have contagious illnesses.
- Requires frequent walking, communication, and interaction with residents, families, and staff.
- Occasional lifting of files, bending, and standing.
Position Type & Schedule:
- Full-time leadership position in a facility operating 24/7.
- Evening, weekend, holiday, and inclement-weather coverage may be required.
Qualifications:
Preferred:
- Bachelor’s degree in Social Work (BSW) or related human services field.
- Minimum of two (2) years’ experience as a Social Worker, preferably in long-term care.

Pay: $41,600.00 - $65,936.00 per year
