Treatment Coordinator

Job Summary

Treatment Coordinator's are responsible for all coordination and planning needed to meet the needs of children in treatment foster care, admitted to Red Mountain Family Services, Inc. Treatment Coordinator's work under the direct supervision of the Red Mountain Family Services Clinical Director and in conjunction with each child's treatment team members.

Responsibilities and Duties

Key responsibilities include:

• The treatment coordinator prepares the initial and comprehensive treatment team plans in accordance with the timelines established in the LCA certification requirements.
• The treatment coordinator coordinates the implementation of the treatment plan.
• The treatment coordinator monitors the client and his/her situation for events related to the treatment team plan or otherwise significant provision of treatment.
• The treatment coordinator documents revisions to the treatment plan
• The treatment coordinator assures that all members of the treatment team plan, including the client as clinically indicated, participate in the treatment team planning process, as documented by the signatures of treatment team members on the treatment planning documents.
• The treatment coordinator involves the client's parents and or legal guardians in treatment team meetings and in all plans and decisions affecting the client and keeps them informed of the client's progress in the program unless prohibited by the court or otherwise contraindicated according to documentation in the client's record
• The treatment coordinator must have a private face to face visit with each client with in the first two weeks of placement and at least twice monthly thereafter. These contacts are conducted both in home and out of home.
• The treatment coordinator has a face to face interview with the client's treatment foster parent with in the first two weeks of placement and at least twice monthly thereafter. The treatment coordinator has a minimum of one phone contact with the treatment foster parents weekly. The phone contact is not necessary in the same week that face to face contact has been made.
• The treatment coordinator documents all contact's in the client's record and include a summary related to the treatment plan, significant events and the communications between treatment coordinators, client, treatment parent and the biological/adoptive parent. All documentation includes the date, time, and location of the contact and names of the persons present.
• The treatment coordinator provides support to client’s relationship with his and or her biological/adoptive family.
• The RMFS treatment Coordinator will complete monthly utilization reviews for each treatment foster child on their case load. The completed utilization review document is to be reviewed by their supervisor or his/her designee before submitting the document. The treatment coordinator is expected to make sure that all of their utilization reviews documents are submitted on time so that there is no loss in funding.

Qualifications and Skills

All potential candidates must have skills and knowledge in principals and theories of social work and/or case management. All candidates must have good verbal skills and writing skills. Must possess a Bachelors degree in Social Work or another related human services field with at least two years experience working with this population. A Masters degree from an accredited Social Work program or other human services field is also acceptable.
