Director of Social Work

Recruit Secure is collaborating with a beautiful Skilled Nursing Facility (SNF) located in Oceanside, NY that is seeking a dynamic and energetic individual to take on the full-time role of Director of Social Services. Responsibilities:

• Develops the Social Services plan of care for residents, identifying goals and approaches and participating in inter-department care planning meetings.
• Participates in Quality processes of the SNF
• Counseling residents and family members; facilitating family meetings
• Complete Admissions, Psychosocial Assessments, and Advanced Directives and assures placement in medical records.
• Oversees referrals for health services and facilitates discharge planning process for the department.
• Assures that policies and procedures are adhered to for the SNF; makes recommendations, updates and revisions to policies when and if needed.
• Completing all required documentation for medical records; producing monthly departmental reports for the SNF Administrator.

Requirements:

• Master’s degree in Social Work, LMSW preferred but not mandatory
• Must have at least 3 years of experience working as a Licensed Social Worker within a Nursing or Rehabilitation Facility.
• At least 1 year of experience in a Senior role
• Should have a good working knowledge of available social services and other community resources
• Must have excellent communication
