Recovery Resource Council is looking for a conscientious, detail-oriented, and compassionate individual who is capable of successfully connecting Veterans to VA health care benefits or community health services where Veterans are not eligible for VA care. The SSVF Healthcare Navigator (HCN) must be able to provide case management and care coordination and work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.Competencies, Knowledge, and Qualifications:
- Ability to work one on one or with large groups
- Ability to work independently and as part of an interdisciplinary team
- Ability to plan, organize, set objectives, time frames, and prioritize
- Skill in conducting client assessments
- Skill in formulating client goals and objectives
- Ability to write clear, concise case notes
- Strong communication skills both orally and in writing
- Knowledge of substance use disorders and mental health issues
- Knowledge of community resources
- Ability to work flexible schedules
- Knowledge of community resources and ability to network
- Understanding of and willingness to practice trauma informed care
- Experience working with veterans is a plus
