What are the responsibilities and job description for the Case Manager position at Real House Inc?
Real House Inc. is seeking a compassionate and organized Case Manager to support clients experiencing mental health, substance use, and co-occurring disorders. The Case Manager will maintain a caseload of approximately 15–20 clients, conduct intake interviews, develop service connections, maintain client records, and coordinate care with clinical staff, healthcare providers, legal representatives, probation and parole departments, and community agencies.
Additional responsibilities include helping clients access housing, healthcare, financial assistance, identification documents, education, and other community resources; providing crisis intervention; facilitating rehabilitation and wellness workshops; completing required documentation and monthly reports; and contributing to a team that provides 24/7 client support.
Candidates must have a high school diploma, strong communication and computer skills, a valid driver’s license, and the ability to work effectively with diverse populations. Experience with the criminal justice system is preferred. Applicants must be able to lift up to 35 pounds, pass a background check, and work weekends and holidays as needed. Individuals in recovery must have five to six years of sustained recovery.
