The Ready To Work (RTW) program opens the doors of hope, transformation, and stability to formerly homeless and/or incarcerated men – rebuilding, restoring and revitalizing their lives. At Ready To Work we seek to address the core of the social, economic, or addiction issues that brought a participant to the program. With shelter, food, and basic safety issues being met, personal development and individual needs can be assessed, the restoration process may begin.

About the Position: The Case Manager is responsible for encouraging, guiding, supporting and monitoring the success of residents at the Ready to Work campus—primarily justice-impacted and homeless men transitioning back into the community. The Case Manager delivers services with a client-first focus within a team setting. This position is responsible for supporting client progress, coordinating with workforce development partners, monitoring compliance (including probation-related obligations), and connecting clients to internal and external resources.
Duties:
Client Intake & Needs Assessment
· Conduct comprehensive assessments for justice-involved individuals to identify needs related to housing, employment, mental health, substance use, and other re-entry barriers—particularly for those experiencing or at risk of homelessness.
· Develop Individual service plans that include, measurable goals in areas such as employment, housing, education, behavioral health, and legal obligations
· Maintain updated records and case plans in accordance with agency standards and funder requirements.
· Provide regular one-on-one meetings with clients to assess progress, offer guidance, and revise goals or strategies as needed.
· Support clients in navigating the criminal justice system, especially those on probation, and assist with court or probation compliance (e.g., check-ins, classes, court appearances).
Re-entry & Housing Stability Planning: Develop individualized re-entry and housing plans tailored to each client’s circumstances, with a focus on stabilization and reducing recidivism.
Resource Navigation & Referrals: Connect clients with community-based services including emergency shelter, transitional housing, permanent supportive housing, workforce development, behavioral health care, and public benefits (e.g., CalWORKs, SSI, Medi-Cal).
Systems Collaboration: Coordinate services and maintain communication with parole/probation officers, correctional institutions, homeless outreach teams, and service providers to ensure continuity of care and compliance with supervision terms.
Client Advocacy: Advocate for clients in navigating systems (housing authorities, courts, employers, service providers), addressing stigma and systemic barriers that affect unhoused populations.
Crisis Management & Stabilization: Respond to client crises including housing loss, mental health episodes, or re-incarceration risk. Provide stabilization services and linkages to immediate supports.
Case Documentation & Compliance:
· Maintain accurate and confidential case notes, service plans, progress reports, and data tracking per grant funding guidelines.
· Prepare reports or summaries as required for internal team meetings, funding requirements, or compliance monitoring.
· Monitor client behavior and participation in the program, reporting concerns to appropriate staff or Probation Officer, and participating in team-based problem solving.
Program Participation & Improvement: Engage in multidisciplinary team meetings, trainings, and strategic planning to support trauma-informed, client-centered re-entry services.
Attend Re-Entry events as needed

Driving: Must maintain a CA driver’s license and be insured to drive in California.
