RN - Per Diem Medicare Home Health Visits - Palm Beach County

Duties

• Conduct thorough patient assessments during Medicare home visits, including vital signs, physical examinations, and evaluation of clinical status.
• Develop and implement individualized care plans aligned with patient needs and healthcare regulations.
• Manage post-acute care transitions by coordinating discharge planning and ensuring proper follow-up care.
• Document all patient interactions accurately using Electronic Health Records (EHR) systems, maintaining compliance with HIPAA and medical terminology standards.
• Collaborate closely with healthcare professionals such as physicians, therapists, and case managers to optimize patient outcomes.
• Provide education on medication management, disease processes, and health maintenance tailored to geriatric, behavioral health, or chronic disease populations.
• Perform diagnostic evaluations and manage complex cases involving infectious diseases or acute conditions in the home setting.

Skills

• Proven critical care experience with a strong background in hospital medicine, ICU, emergency medicine, or trauma centers.
• Experience working within Level I or Level II trauma centers and managing patients in emergency room or inpatient settings.
• Knowledge of infectious disease care, post-acute care protocols, hospice services, and discharge planning procedures.
• Skilled in utilizing EMR/EHR platforms for documentation and managing medical records efficiently.
• Excellent patient service skills with the ability to communicate effectively with diverse client populations including pediatrics and geriatrics.
• Strong understanding of physiology, medical terminology, HIPAA regulations, and healthcare policies related to managed care and utilization management.
• Ability to perform comprehensive patient assessments, vital signs monitoring, case management activities, and diagnostic evaluations.
• Experience in outpatient clinics or home health environments is highly desirable for delivering seamless outpatient and post-discharge care.
