Ready and Willing 2 Help Health Care is seeking a Registered Nurse (RN) Field Nurse to join our growing home health team. This position is ideal for an RN who is confident working independently in patient homes, values strong clinical documentation, and is ready to grow with an expanding agency.
We want to be transparent: this role will begin with variable hours, as we are currently building our active census. We believe the schedule will grow to more than 20 hours per week, with strong potential to move into full-time hours and possible overtime as admissions increase.

Responsibilities:
- Provide skilled nursing services in patients’ homes
- Complete initial and ongoing nursing assessments
- Perform and document OASIS, skilled nursing notes, and other required clinical documentation
- Develop, update, and communicate individualized plans of care
- Monitor patient condition and report changes to the physician and care team
- Educate patients and families on care needs and treatment plans
- Coordinate services with physicians, caregivers, and other disciplines
- Evaluate ADL/iADL needs and determine whether home health aide services are appropriate
- Supervise home health aides in accordance with agency policy and state/federal requirements
- Submit re-authorization requests for additional visits when needed
- Participate in case conferences, staff meetings, in-services, and quality assurance activities
- Complete timely clinical record reviews and maintain compliance with agency standards
- Travel to patient residences for visits and assessments
Qualifications:
- Graduate of an approved professional nursing program
- Current Indiana RN license in good standing
- Minimum 1 year of RN experience required
- Home care or home health experience preferred
- Current CPR certification
- Valid driver’s license, reliable transportation, and ability to travel to patient homes
- Strong assessment, documentation, communication, and time-management skills
- Ability to work independently in the field while collaborating with leadership and the care team
Work Environment:
- Patient care is provided in patient residences
- Frequent travel between homes and meetings
- Possible exposure to communicable diseases
- Must be able to access homes that may not be handicap accessible
- Must be able to perform the physical demands of field nursing

Transparency About Hours
This is not a guaranteed 40-hour role on day one. We are intentionally being upfront that hours will vary at the start. That said, we have a strong admission pipeline and expect this position to grow quickly for the right nurse.
Pay: $30.00 - $34.00 per hour
