Medication Collection Technician

Job Summary

Under the general direction of the hospital pharmacy, Medication Collection Pharmacy Technicians:

• Independently perform tasks related to collecting, reconciling, and entering the home medication history into the patient's electronic medical record.
• Interview the patient, patient family members, call local pharmacies and use other available resources for the purpose of compiling a medication history.
• Compile and enter medication history information into the patient's electronic health record for reconciliation by the admitting provider and use by physicians and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital.
• Interact with and provide medication history and other related patient information to nurses, physicians, pharmacists, and other medical professionals as appropriate. Improve safety and quality of our patients by providing physicians and physician extenders with a comprehensive home medication list.
• May perform a variety of functions which impact the medication reconciliation process and patient care.
• Assists the pharmacist in his/her professional responsibilities in providing pharmacy services.
• Working under the direction of a licensed pharmacist, participates in the distribution of medications and performs tasks associated with regulatory compliance.

Qualifications

Education Requirements

• Completes and maintains all competencies and trainings as required for role.
• High School Diploma - OR - GED
  
  

Experience

• Retail Pharmacy Experience Preferred
  
  

Certification and Licensure

• National Pharmacy Technician Certification (CPhT) through the Pharmacy Technician Certification Board (PTCB) required upon hire or within the first 12 months of employment
