Position Summary:

As a Case Manager at REACH For Tomorrow Inc, your primary responsibility is to provide comprehensive support and guidance to individuals who are transitioning from a rehabilitation facility to independent living while recovering from substance use disorder. You will work closely with clients to assess their needs, develop personalized recovery plans, and connect them with appropriate resources and services. Your role is crucial in assisting clients in achieving long-term sobriety, stability, and successful reintegration into society.


Key Responsibilities:


Client Assessment and Goal Setting:

  • Conduct initial assessments to evaluate clients' addiction history, mental health status, support systems, and overall well-being.
  • Collaborate with clients to establish achievable short-term and long-term recovery goals.
  • Continuously monitor clients' progress, reassess goals as needed, and adapt plans accordingly.
  • Individualized Treatment Planning
  • Develop personalized recovery plans in collaboration with clients, taking into consideration their unique needs, strengths, and challenges.
  • Identify appropriate services and resources, including counseling, medical treatment, employment assistance, housing options, and social support networks.
  • Coordinate with internal and external stakeholders to ensure clients receive timely and comprehensive care.


Counseling and Support:

  • Provide individual and group counseling to clients, offering guidance and encouragement throughout their recovery journey.
  • Address clients' emotional, behavioral, and social needs, facilitating their development of healthy coping mechanisms and relapse prevention strategies.
  • Conduct regular check-ins and follow-ups to monitor clients' well-being and provide ongoing support.


Referral and Resource Coordination:

  • Collaborate with community organizations, healthcare providers, and social service agencies to connect clients with appropriate resources and support systems.
  • Assist clients in navigating the healthcare system, securing health insurance, and accessing medical and psychiatric services.
  • Coordinate referrals for vocational training, educational opportunities, housing assistance, and other relevant programs.

 

Documentation and Reporting:

  • Maintain accurate and up-to-date client records, documenting assessments, treatment plans, progress notes, and any relevant interactions or services provided.
  • Prepare reports and maintain statistics to track program outcomes, trends, and areas for improvement.
  • Ensure compliance with confidentiality guidelines and legal requirements for recordkeeping.

 

Team Collaboration:

  • Collaborate effectively with interdisciplinary teams, including therapists, physicians, social workers, and other professionals, to ensure coordinated and holistic care for clients.
  • Participate in team meetings, case conferences, and staff development activities to enhance knowledge and skills related to addiction treatment and recovery.


Qualifications and Skills:

  • A valid US Drivers License with a clean driving record history.
  • Relevant experience in addiction recovery, substance abuse counseling, or mental health services.
  • Strong understanding of addiction, recovery principles, and evidence-based treatment approaches.
  • Knowledge of community resources, support services, and treatment options for individuals recovering from addiction.
  • Excellent communication skills, both verbal and written, with the ability to build rapport and establish therapeutic relationships.
  • Empathy, compassion, and non-judgmental attitude toward individuals struggling with addiction.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage a caseload effectively.
  • Familiarity with documentation and recordkeeping requirements in compliance with relevant regulations and best practices.
  • Ability to work collaboratively in a team environment and engage in multidisciplinary care planning.
  • Cultural sensitivity and the ability to work effectively with individuals from diverse backgrounds.
