Your Role and Impact
As a Telehealth Dietitian, you’ll play a pivotal role in
delivering individualized nutrition care to patients from the comfort of your
home. Your expertise will help people lead healthier lives and manage chronic
conditions effectively.
Responsibilities:
- Conduct
virtual nutritional assessments of patients via telehealth platforms
(e.g., Teams, Google Meet, Zoom, FaceTime, phone calls).
- Evaluate
nutritional risk factors and craft personalized nutrition care plans to
manage chronic conditions, prevent illness, and maintain comfort.
- Review
patient information, including H&P/face sheets, current progress
notes, intake, anthropometrics, etc., provided by the clinical team.
- Complete
comprehensive and timely progress notes according to the provided
template.
- Collaborate
effectively with the interdisciplinary care team to ensure coordinated
patient care.
- Stay
current on best practices in nutrition care and telehealth.
Experience & Qualifications:
- Bachelor's
or Master's degree in Dietetics, Nutrition, or a related field from an
accredited program.
- Registered
by the Commission on Dietetic Registration.
- Current
state licensure/certification (if mandated by the state)
- Minimum
of 1 year clinical experience (preferred).
- Excellent
communication skills for effective interaction with patients, families,
and healthcare teams.
- Possesses
the ability to apply critical thinking to a variety of situations, assess
objectives, develop and implement and monitor effective plans for
improvement.
- Proficiency
in providing general nutrition interventions.
- Competency
in electronic charting systems
