About the Role:
Are you a patient-centered healthcare professional committed to delivering high-quality, respectful care to a diverse patient population? We are seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join our collaborative, multidisciplinary team.
As a PCP, you key responsibilities will include:
Clinical Care
- Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan.
- Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines.
- Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals.
- Document all care promptly in the electronic health record (EPIC).
- Provide primary care services
Collaboration & Coordination
- Make appropriate internal and external referrals.
- Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care.
Quality & Professional Development
- Participate in quality improvement initiatives and case conferences.
- Engage in ongoing professional development opportunities, including continuing medical education.
Work Schedule:
- Four morning/daytime shifts: 8:30 AM – 4:30 PM
- One evening shift: 12:00 PM – 8:00 PM
