Absolut Care of Gasport is seeking an experienced MDS RN to join our team. The MDS RN plays a key role in ensuring accurate resident assessments, strong clinical outcomes, and compliance with all state and federal guidelines.
Responsibilities:
- Complete, manage, and submit MDS assessments on time
- Develop and update individualized care plans
- Partner with nursing, therapy, and interdisciplinary teams
- Support survey readiness and quality initiatives
- Educate staff on documentation requirements
Requirements:
- Current NYS RN license
- Previous MDS/RAI experience required
- Knowledge of PDPM, care planning, and clinical documentation
- Strong organizational and communication skills
