Key Responsibilities
- Assist with clinical coordination and office management under RN supervision
- Support case management and contribute to the development and implementation of individualized care plans
- Complete and update HRST assessments, protocols, and documentation
- Provide clinical support and guidance to field nurses and direct care staff
- Participate in leadership functions, including staff training, quality assurance, and compliance monitoring
- Collaborate with interdisciplinary teams to ensure continuity and quality of care
- Active LPN license in the state of [Insert State]
- Minimum 4 years of LPN experience, including roles such as Unit Manager, ADON, or similar leadership positions
- Background in homecare, preferably with experience in IDD/DD populations
- Familiarity with DBHDD regulations, HRST tools, and person-centered planning. Willing to train the right candidate
- Strong documentation, communication, and organizational skills
- Interest in learning the business and operational aspects of homecare services
- Confident clinical judgment and ability to support field staff
- Proactive, detail-oriented, and adaptable to changing priorities
