Ravenwood Health is hiring a CME Outreach and Intake Specialist.
- Detail-oriented, organized and able to synthesize information from multiple sources.
- Effective client service, communication and collaboration skills required.

- Strong written and verbal communication skills.
- Actively outreaches all new referrals to determine client/family interest in OhioRISE and care coordination services.
- Completes CANS assessments for referrals as needed.
- Actively communicates with referents to obtain collateral information and provide referral status updates.
- Ensures warm hand-off to assigned care coordinator.
- Has experience with electronic healthcare systems and general office technology.
- Is effective in establishing rapport and building a collaborative relationship with youth and their families/caregivers.
- Displays professionalism, maturity, and respect in interactions with clients and their families.
- Demonstrates good decision making and problem-solving skills.
- Employees demonstrate strong communications skills by returning and responding to phone calls or emails within 24 hours and other correspondence in a timely manner, and through their ability to interact across cultures.
- Keeping appropriate professional boundaries and seeking supervision as needed.
- Has knowledge of clinical needs of children, youth, and families.

Required Education and Experience
Have a minimum of three years' experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field, providing community-based services to children and youth and their family or caregivers.
Preferred Education and Experience
Background and/or experience in one or more of the following: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
Cultural knowledge of key populations that will be served.

While performing the duties of this job, the employee is regularly required to communicate with youth and their families/caregivers, as well as collaterals and community partners.
Some in person travel within the catchment area is necessary to complete CANS assessments.
