Job Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team as a Staff Nurse. The ideal candidate will provide high-quality patient care across various settings, including acute care, long-term care, hospice, and community health environments. This role requires a compassionate approach, strong clinical skills, and the ability to work effectively with diverse patient populations, including seniors, individuals with developmental disabilities, and those requiring specialized care such as Alzheimer's or memory care. The LPN will utilize their expertise in medical documentation, patient assessment, medication administration, and vital signs monitoring to support patient recovery and well-being.
Duties
- Conduct comprehensive physical examinations and patient assessments using physiology knowledge and medical terminology.
- Administer medications via injections, IV infusion, venipuncture, and catheter care while adhering to aseptic techniques and infection control protocols.
- Manage airway and ventilator support for patients requiring respiratory assistance.
- Document medical records accurately using EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Assist with diagnostic evaluations including laboratory specimen collection/processing, sonography, and medical imaging procedures.
- Provide specialized care for patients with complex needs such as acute pain management, dementia care, hospice care, or post-acute recovery.
- Support discharge planning and coordinate with case management teams to ensure seamless patient transitions across inpatient and outpatient settings.
- Perform basic life support (BLS), sterile processing, suturing, and other advanced procedures as required.
- Educate patients and families on health coaching, nutrition, medication management, and disease prevention strategies.
- Maintain compliance with HIPAA regulations and NCQA standards to ensure confidentiality and quality of care.
Experience
Candidates should possess extensive experience in various healthcare environments including hospital medicine, ICU/CCU/PICU/Neonatal ventilator management, emergency room or urgent care settings, outpatient clinics, or home & community care. Prior experience working with special populations such as pediatrics, geriatrics—including dementia or Alzheimer's care—behavioral health patients—and individuals with disabilities is highly valued. Proficiency in ICD-9/10 coding, CPT coding, medical documentation software like EMR/EHR systems (Epic preferred), and familiarity with infection control protocols are essential. Additional desirable skills include experience in dialysis, sterile processing, laboratory work, spinal tap procedures, gastric lavage, dermal fillers administration (for aesthetic practices), occupational health services, and travel nursing assignments.
