You value collaboration and build strong relationships with care teams - including PCPs, HHAs, and others; to ensure best practices are followed and outcomes extend beyond wound care.
What will you do?
- Travel to patients’ residences, assisted living, and skilled nursing facilities within the Triad area
- Perform comprehensive wound assessments and develop individualized treatment plans
- Diagnose and manage both acute and chronic wounds using advanced modalities
- Order and interpret diagnostic tests as appropriate
- Prescribe medications and implement evidence-based treatments to promote healing
- Educate patients, caregivers, and facility staff on wound care best practices
- Collaborate with our team and referring providers to ensure continuity of care
- Maintain accurate, timely documentation in our EMR system
What will you need?
- Master’s degree from an accredited NP or PA program
- Board certification (Nurse Practitioner or Physician Assistant)
- Active North Carolina License
- Experience in wound care (clinic, hospital, or post-acute setting)
- Knowledge of LCD medical documentation requirements
- Reliable transportation for patient visits
- Excellent communication skills and compassionate bedside manner
- Strong organizational skills and attention to detail
- Familiarity with post-acute settings (ALFs/SNFs) is a plus
- Proficiency with EMRs and infection control standards
- Comfortable with HHA processes and protocols
Preferred
- Wound care certification (CWS,WCC, CWCN, etc.)
- Knowledge of skin substitutes and advanced wound therapies
- Familiarity with AdvancedMD EMR
