A respected skilled nursing and rehabilitation facility in Rochester, NY, is seeking an experienced and detail-oriented MDS Coordinator (RN) to join our clinical team. We are looking for a strong RN who understands accurate assessment, proper reimbursement, and how to drive quality outcomes through the MDS process.
Key Responsibilities
Complete and manage MDS assessments, care plans, and RAI processes according to regulatory timelines.
Ensure accuracy of diagnoses, documentation, and coding to support proper PDPM reimbursement.
Collaborate with nursing, therapy, dietary, and social work to gather supporting information.
Participate in care plan meetings and quality improvement initiatives.
Audit records, monitor compliance, and assist with survey readiness.
Work closely with the DON and Administrator to drive clinical and financial outcomes.
Qualifications
Active RN license in New York (required).
Prior MDS/RAI experience in a skilled nursing facility required.
Strong understanding of PDPM, ICD-10 coding, and care plan development.
Excellent organizational skills and attention to detail.
Ability to communicate and collaborate with multiple departments.
