Role Description This is a full-time, on-site Nurse Practitioner role located in Norwood, MA. The Nurse Practitioner will provide direct patient care, including health assessments, diagnosis, treatment, and follow-up for preventive, acute, and chronic conditions in accordance with evidence-based guidelines. Daily responsibilities include conducting patient examinations, ordering and interpreting diagnostic tests, prescribing medications, developing care plans, and monitoring outcomes. The role involves patient education on disease prevention, medication adherence, and lifestyle modification, as well as coordinating care with physicians, specialists, and other healthcare professionals. The Nurse Practitioner will document all encounters in the medical record, participate in quality improvement efforts, and support clinic workflows to ensure timely, efficient, and safe care delivery.

Qualifications

  • Active Nurse Practitioner license in the state of Massachusetts and national board certification (e.g., FNP, AGNP, or comparable).
  • Demonstrated experience in primary care or internal medicine, including management of chronic diseases such as diabetes, hypertension, and cardiovascular conditions.
  • Strong clinical assessment, diagnostic, and treatment-planning skills, with the ability to apply evidence-based guidelines in day-to-day practice.
  • Excellent patient communication and education skills, with the ability to explain complex medical information in an understandable and culturally sensitive manner.
  • Ability to work collaboratively within a multidisciplinary care team and to coordinate care across different providers and services.
  • Proficiency with electronic health records (EHR) systems and accurate clinical documentation practices.
  • Strong organizational and time-management skills, with the ability to manage a full patient panel and prioritize tasks in a busy clinical setting.
  • Commitment to continuous professional development, quality improvement, and adherence to ethical and regulatory standards.
  • Master’s degree in Nursing from an accredited program; experience in outpatient clinic settings is preferred.
