What are the responsibilities and job description for the Home Health Case Manager position at Rainbow Home Health?
- Patient assessment: Evaluate the patient's physical, mental, and emotional needs and wellness.
- Care plan development: Create, implement, and adjust an individualized Plan of Care (POC) in collaboration with the patient, family, and healthcare team.
- Coordination of services: Coordinate with physicians, nurses, therapists (physical, occupational), home health aides, and other external healthcare providers to implement the care plan.
- Patient and family support: Provide education, support, and counseling to the patient and their family regarding the health condition and care plan.
- Monitoring and evaluation: Regularly monitor patient progress and compliance with the treatment plan, adjusting the plan as needed to improve outcomes.
- Documentation: Complete and maintain accurate clinical records and case documentation.
- Communication: Act as a liaison between the patient, family, physicians, and insurance companies.
- Supervision: Supervise and guide other home health staff, such as home health aides, to ensure they are following the plan of care.
