You Will:
- Provide leadership, direction, and direct supervision to Population Health Coordinators, Navigators, and program staff.
- Oversee day-to-day program operations, ensuring staff performance aligns with operational goals, productivity standards, and patient care objectives.
- Lead staff development through coaching, training, performance feedback, and ongoing professional development initiatives.
- Conduct regular team meetings and one-on-one meetings to evaluate workloads, performance outcomes, and opportunities for operational improvement.
- Serve as a point of escalation for complex operational issues or patient care coordination challenges requiring managerial review.
- Ensure programs operate efficiently through collaboration with physicians, clinical teams, and operational leadership.
- Provide staff guidance on workflow management, case coordination challenges, and operational problem-solving.
- Maintain collaborative partnerships with physicians, clinical departments, and operational leadership to ensure alignment of population health initiatives with organizational priorities.
- Facilitate cross-departmental communication and coordination to support patient care initiatives and program success.
- Oversee program documentation, reporting processes, and data tracking systems to ensure accuracy and compliance.
- Monitor dashboards and reporting tools to evaluate program performance and operational effectiveness.
- Prepare and present performance reports, operational summaries, and program updates for leadership review.

To Ensure Success In This Role, You Must Have:
- Certification as Breast Navigator
- 3–5 years of experience in healthcare operations, care coordination, population health, or program management.
- Supervisory or leadership experience preferred.
- Strong verbal and written communication skills.
- Experience with Microsoft Office, Google Docs, Excel, and program tracking systems.
- Strong organizational skills and ability to manage multiple priorities.
- Ability to work both independently and collaboratively with multidisciplinary healthcare teams.
