Radiant Health is looking for a compassionate and dedicated Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team.

This is an opportunity to use your nursing skills in a community-based behavioral health setting where your work can have a direct and lasting impact on individuals living with serious mental illness and substance use disorders.

As an ACT RN, you’ll work closely with an interdisciplinary team to provide nursing care, medication support, health education, care coordination, and ongoing assessment — helping clients achieve greater stability, independence, and wellness in their own communities.

What You'll Do

• Conduct comprehensive assessments of clients’ physical, psychiatric, medical, and social needs.
• Monitor mental health symptoms, medication effectiveness, side effects, and adherence concerns.
• Administer long-acting psychotropic medications via IM injection in community settings, including correctional facilities when applicable.
• Complete medication reconciliation and coordinate medication deliveries and refills.
• Assess and triage mental health and physical health concerns and coordinate appropriate care.
• Review laboratory results and ensure appropriate follow-up.
• Provide medication education and help clients understand their treatment and wellness needs.
• Coordinate healthcare services and communicate with providers, pharmacies, hospitals, and community resources.
• Collaborate closely with psychiatrists, case managers, therapists, peer support specialists, and other members of the ACT team.
• Build trusting relationships with clients while supporting their goals for recovery, stability, and improved quality of life.
• Provide services in the community, meeting clients where they are rather than limiting care to a traditional office setting.

Professional:

• Graduate of an accredited school of nursing.
• Licensed as a Registered Nurse in the State of Indiana.
• Ability to understand all Federal, State, and Local regulations that are applicable to the practice of Nursing.
• Negative Mantoux.
• Has or be willing to obtain CPR Certification.
• Has or be willing to obtain Crisis Prevention & Intervention Certification.
• Must be able to hear, see, speak and be ambulatory for observation and communication regarding patient care.
