Radial Health is a leader in Physical Medicine and Rehabilitation, providing services throughout the continuum. We are expanding our presence throughout the United States. Our highly specialized PM&R teams and integrated services help patients to regain their functional abilities and assists in managing care across the healthcare continuum.
We are seeking a NP/PA to join our team of exceptional providers in Lakeland, FL. You will work collaboratively with an experienced PM&R team to manage medical and rehab care on inpatient rehab units, LTACHs, skilled nursing facilities, and Assisted Living Facilities. The core duties will include rounding on patients daily in a team-based model, being a leader in weekly interdisciplinary team rounds, supporting the overall care plan as written by the physician, respond to calls, managing patients through the continuum, providing education to patients and staff, and supporting smooth and timely discharges.
Qualifications:
- Experience in physical medicine and rehabilitation is preferred, but not required.
- Experience in orthopedic and/or neurology preferred.
- Graduate of an accredited NP/PA Program
- Certification as an NP/PA and licensed by the State of Florida.
- Candidates MUST be eligible to apply or have a Controlled Substance License and DEA.
- Excellent clinical skills, interpersonal relationships, and organizational ability.
