Job Description:
● Provided direct nursing care to residents in long-term care and skilled nursing facilities
● Administered medications, treatments, and injections as prescribed by physicians
● Monitored residents’ vital signs, health conditions, and changes in behavior
● Assisted residents with daily living activities and ensured comfort and safety
● Maintained accurate patient records, charts, and documentation in EMR systems
● Collaborated with RNs, physicians, and healthcare teams to develop patient care plans
● Performed wound care, catheter care, and other nursing procedures as required
● Responded promptly to emergencies and resident care needs
● Supervised CNAs and assisted nursing staff in daily patient care activities
● Educated residents and family members regarding treatment plans and healthcare needs
● Ensured compliance with healthcare regulations, infection control, and safety standards
● Supported rehabilitation and restorative care programs to improve resident well-being
