SummaryPart-time position serving children infants/toddlers with disabilities in the Birth to Three Program in Racine County. The social worker/service coordinator will be a member of an Early Intervention Team supporting infants and toddlers with developmental delays and their families in the child’s natural environment. The service coordinator is responsible for coordinating the intake and evaluations, development of IFSP, and assisting and enabling eligible children, birth to three years old, and their family's to receive early intervention services. In addition, the service coordinator assists the family in the transition process to services after three years of age, if needed. Bilingual skills are a plus.Bachelor’s degree in human services field or related field and preferable experience working with individuals with developmental disabilities/delays in early intervention setting.
