Why this role matters

  • Be the on-the-ground support helping adults with complex mental health/substance-use needs build skills and stability in real-life settings.
  • Keep services moving across home, school, work, and community—so people can practice skills where they live and learn.
  • Contribute to a 24/7/365 team that can respond, de-escalate, and keep care connected.

What you’ll do

  • Psychoeducation & skills: Provide psychoeducation and structured skill-building/symptom-management as outlined in the Person-Centered Plan (PCP).
  • Crisis first responder: Assist with de-escalation and crisis stabilization; follow crisis plans and safety protocols.
  • Plan execution: Collaborate with the Team Lead/QP on the development, implementation, and revisions of PCPs.
  • Behavioral & SUD supports: Implement behavioral and substance-use interventions under Team Lead direction; support relapse-prevention/disease-management strategies.
  • Resource coordination: Facilitate linkage and referrals to formal/informal supports (housing, benefits, transportation, employment, recovery supports).
  • Progress monitoring: Track engagement and goals; communicate effectiveness and barriers to the Team Lead; provide follow-up as needed.
  • Documentation: Complete accurate, timely notes and incident reports consistent with agency policy and state rules.

You’ll thrive here if you…

  • Enjoy field-based work across homes, workplaces, and community settings.
  • Stay calm in fast-moving situations and use clear, compassionate communication.
  • Are coachable, organized, and team-oriented—comfortable taking initiative and following through.
  • Practice cultural humility, maintain boundaries, and protect confidentiality.
