Job Overview
Join a leading acute care facility as a Registered Nurse (RN) Case Manager and play a critical role in coordinating patient care and discharge planning. In this position, you will work closely with multidisciplinary teams to ensure patients receive appropriate, efficient, and high-quality care throughout their hospital stay and transition to post-acute settings.
Responsibilities
- Perform comprehensive case management and discharge planning for patients
- Coordinate care across multidisciplinary teams to ensure optimal outcomes
- Conduct utilization reviews and determine medical necessity using evidence-based guidelines
- Manage care plans, including DME needs and disease management
- Assist with admission criteria, prior authorizations, and benefits eligibility
- Ensure compliance with CMS, DRG, and HIPAA regulations
- Maintain accurate documentation in Epic EMR system
- Support patients and families with education and care coordination
Requirements
- Active California RN License (required; must be active by start date)
- Minimum 2 years of acute care case management / discharge planning experience
- BLS Certification required
- Experience with Epic charting system
- Travel experience required (first-time travelers may be considered with screening)
- Strong knowledge of utilization review, InterQual criteria, and care coordination
Schedule & Additional Details
- Shift: Day shift
- Patient Ratio: 1:20
- Weekend Requirement: Every other weekend holidays
- Floating: Not required
- RTO: Maximum 7 days
