Responsibilities

I. JOB SUMMARY/RESPONSIBILITIES:
• Under the general supervision of the Clinical Manager or the designee, assesses, plans, implements, coordinates, monitors and evaluates options and services required to meet the needs of patients to promote quality cost-effective care by helping the Medicare Certified Services (MC) staff.
• Ensures that specific outcomes are achieved and that variances from established and accepted parameters are evaluated and addressed, as necessary.
• The Team Leader is knowledgeable in community health nursing and/or rehabilitation concepts and can work independently and communicate effectively with clients, staff, and providers. Providing lead direction to professional, technical and support staff, and providing case management services for CareResource Hawaii (CRH) patient.
II. TYPICAL PHYSICAL DEMANDS:
• Sitting, finger dexterity, seeing, hearing, speaking.
• Constant: reaching above, at and below shoulders; repetitive arm/hand motion and frequent gripping of objects.
• Frequent: lifting maximum weight up to 50 pounds, carrying usual weight of 20 pounds, pushing/pulling maximum weight of 50 pounds of force, standing, walking, stooping/bending, and twisting body.
III. TYPICAL WORKING CONDITIONS:
• Subjected to adverse environmental conditions like continuous exposure to blood borne pathogens.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Associate’s or Bachelor's degree in Nursing; OR Bachelor's degree in Occupational Therapy; OR Master's or Doctorate in Physical Therapy; OR Master's degree in Speech Language Pathology.
• Current Hawaii State license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist.
• Valid U.S driver’s license, vehicle and vehicle insurance as required by Hawaii State Law.
• Driver's abstract or evidence of good driving record.
• Current BLS certification.
B. EXPERIENCE:
• One (1) year experience as a Home Health Therapist or one (1) year experience Home Health nurse preferably in a comparable institution.
