We are seeking motivated Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our expanding mobile care team as independent contractors (1099). In this role, you will provide in-home and in-clinic visits, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care. This is an opportunity to practice meaningful, relationship-based medicine while working autonomously in a flexible, supportive environment.
Core Responsibilities
- Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
- Deliver mobile wound care services directly in patients’ homes or care facilities, ensuring comfort, accessibility, and continuity of care.
- Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
- Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
- Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
- Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
- Other duties as assigned.
Qualifications
- Active and unrestricted Nurse Practitioner or Physician Assistant license in Nebraska or Iowa.
- Experience in wound care, home-based care, or geriatric medicine preferred.
- Reliable transportation and valid driver’s license. The position involves day-trip travel to patient homes and partner facilities.
- Strong clinical assessment, communication, and documentation skills.
- Self-starter comfortable working independently with mobile technology.

Salary : $95,049 - $137,523
