Licensed Clinical Social Worker

We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join Quality Medical Associates, a division of Quality Life Services, in a unique clinical leadership role supporting our facilities throughout Western Pennsylvania.

This position combines direct clinical expertise with company-wide social services support. You will work alongside facility social workers, administrators, nursing leadership, providers, and interdisciplinary teams to strengthen psychosocial services, assist with complex resident needs, support regulatory compliance, and promote consistent, person-centered care across the organization.

This is not a remote position. Regular travel to Quality Life Services locations is required, and a company vehicle is provided.

RESPONSIBILITIES

• Provide clinical consultation, guidance, and support to social services staff throughout Quality Life Services.

• Assist with complex resident cases, behavioral health concerns, crisis intervention, ethics consultations, and resident advocacy.

• Conduct comprehensive psychosocial assessments and provide individual, family, and group counseling as clinically appropriate.

• Assess and support residents experiencing depression, anxiety, grief, trauma, adjustment concerns, loneliness, substance use concerns, and other behavioral health needs.

• Develop and help standardize social services policies, procedures, and best practices across QLS facilities.

• Support facility survey readiness, regulatory compliance, quality assurance, and performance improvement initiatives.

• Provide education and training related to psychosocial care, resident rights, trauma-informed care, dementia care, behavioral health, and discharge planning.

• Collaborate with facility administrators, nursing leadership, physicians, nurse practitioners, rehabilitation professionals, social services teams, behavioral health providers, and corporate leadership.

• Support recruiting, mentoring, education, and development of social services staff.

• Assist residents and families with care planning, transitions of care, community resources, advance care planning, conflict resolution, and discharge planning.

• Maintain timely and accurate clinical documentation within PointClickCare (PCC).

• Promote resident dignity, autonomy, emotional well-being, quality of life, and person-centered care.

• Ensure services comply with CMS requirements, Pennsylvania regulations, HIPAA, professional standards, and the NASW Code of Ethics.

REQUIREMENTS

• Current, unrestricted Licensed Clinical Social Worker (LCSW) license in the Commonwealth of Pennsylvania – required.

• Master's degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program.

• One to two years of post-licensure clinical experience preferred.

• Experience in long-term care, skilled nursing, geriatrics, hospice, behavioral health, or another healthcare setting preferred.

• Strong clinical assessment, counseling, crisis intervention, and conflict-resolution skills.

• Knowledge of geriatric mental health and psychosocial issues affecting older adults.

• Knowledge of CMS regulations, HIPAA, and Pennsylvania long-term care requirements.

• Excellent interpersonal, communication, documentation, organizational, and time-management skills.

• Ability to work collaboratively with facility teams, residents, families, healthcare providers, and organizational leadership.

• Experience with PointClickCare (PCC) or other electronic health record systems preferred.

• Valid driver's license and ability to regularly travel among Quality Life Services locations.

Preferred: Certified Dementia Practitioner (CDP), trauma-informed care training, or certification/experience in palliative care, hospice, or geriatric behavioral health.
