RESPONSIBILITIES
- Registered Nurse Care Coordinator (RNCC) position is responsible for the coordination of care and services as well as case management/utilization for the patients that we serve
- Work closely with the facility leadership, direct staff, patients, and their families in addition to the CRNP in the facilities
- Regularly attend calls and meetings within the home
- Attends AM Clinical, Stand Up, Care Conferences, QAPI, PIPs, Risk, and other regularly scheduled team meetings
- Integral role within the ISNP (Institutional Special Needs Plan) and PPHP (Provider Partners Health Plan)
REQUIREMENTS
- Active and good-standing RN license in the state of Pennsylvania
- 5 years of experience, post-acute experience preferred
- Case Management and insurance knowledge preferable
