We are seeking a compassionate, motivated, and dependable Physical Therapist (PT) to provide exceptional care to patients throughout our Northern Utah service area, including Brigham City, Ogden, Clearfield, Layton, Bountiful, and surrounding communities.
This is a pay-per-visit opportunity with mileage reimbursement, ideal for a Physical Therapist who enjoys flexibility, independence, and providing personalized, one-on-one care in the comfort of patients' homes.

Position Overview:
As a Physical Therapist on our team, you will provide skilled physical therapy services to patients in their homes throughout our North Region.
You will work collaboratively with physicians, nurses, and other members of the care team to develop individualized treatment plans that promote safety, mobility, independence, and overall well-being.
We are looking for someone who is passionate about patient care, takes pride in their work, and enjoys building meaningful relationships with patients and families.
Key Responsibilities:
- Complete comprehensive physical therapy evaluations and assessments.
- Develop and implement individualized treatment plans based on patient needs and physician orders.
- Provide skilled physical therapy interventions to improve mobility, strength, balance, and functional independence.
- Monitor patient progress and adjust treatment plans as appropriate.
- Educate patients and caregivers on exercises, safety, mobility, and home programs.
- Collaborate with physicians and interdisciplinary team members to coordinate high-quality patient care.
- Complete accurate and timely clinical documentation in accordance with agency policies and applicable regulations.
- Complete required home health assessments and reassessments, including applicable OASIS documentation.
- Participate in care coordination and quality improvement activities as needed.
Qualifications:
- Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist license in the State of Utah.
- Previous home health experience preferred but not required.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently while maintaining effective communication with the clinical team.
- Compassionate, patient-centered approach to care.
- Reliable transportation and willingness to travel to patients' homes throughout the North Region.
