Position Overview
Quality Healthcare Group, Inc. is seeking an experienced Licensed Speech Therapist (SLP) to provide in-home speech-language and cognitive-communication therapy services to adults, some with traumatic brain injury (TBI), intellectual and developmental disabilities (IDD), and other communication disorders. This is an opportunity to make a direct impact on patients' quality of life within their home environment.
Key Responsibilities:
- Conduct comprehensive evaluations of speech, language, communication, and cognitive-communication skills.
- Develop and implement individualized treatment plans (POCs) in collaboration with physicians, care coordinators, and interdisciplinary team members.
Provide therapy focused on:
Speech production, articulation, and voice.
- Language comprehension and expression.
- Cognitive-communication skills (attention, memory, problem-solving, executive function).
- Swallowing disorders (dysphagia), as appropriate.
- Work with patients with TBI, mental disabilities, autism spectrum disorder, and other neurological/communication challenges.
- Train family members, caregivers, and direct support staff on strategies to support communication and cognitive function.
- Document all evaluations, progress notes, and care coordination in compliance with state, federal, and payer requirements (Medicaid/Medicare/MCOs).
- Participate in care team meetings and maintain communication with physicians, case managers, and agency staff.
- Maintain professional licensure, continuing education, and compliance with HIPAA and agency standards.
Qualifications
- Master's Degree in Speech-Language Pathology from an accredited program or equivalent accreditation
- Current state licensure (Pennsylvania/New Jersey) as a Speech-Language Pathologist.
- Certificate of Clinical Competence (CCC-SLP) from ASHA strongly preferred.
- Minimum 2 years of experience in home health care or community-based settings.
- Demonstrated experience working with patients with TBI and intellectual/mental disabilities.
- Strong knowledge of cognitive rehabilitation strategies, AAC (augmentative/alternative communication), and behavioral support techniques.
- Ability to work independently as a 1099 contractor with strong time-management and documentation skills.
- Must have reliable transportation and willingness to travel to patient homes.
- CPR certification and background checks required.
