Description
POSITION PURPOSE: To perform a variety of clerical duties involved in maintaining patient records (manually and/or electronically), including filing, retrieving, and distributing medical records, loose notes, and reports and maintaining associated logs and records in accordance with established priorities and procedures of the Medical Records Department.
ESSENTIAL FUNCTIONS AND BASIC DUTIES:
- Monitor that all medical records are sorted, filed, collated, and maintained in the appropriate manner.
- Retrieve daily all medical records for next-day scheduled appointments.
- File and retrieve medical records and research lost or missing records in accordance with established procedures.
- Examine patient medical records for completeness or deficiencies, ensuring all required information is included, note any deficiencies, and refer to the appropriate provider for follow-up correction.
- Process all authorized requests for medical records release of information and properly copy all medical information requested.
- Assemble and maintain an adequate amount of new charts.
- Properly document a daily log of medical records incoming and outgoing mail.
- Properly document a daily log of requests for medical records.
- Daily logging and filing of all medical consults, and other medical information to be placed on charts.
- Documentation of missed appointments, and other relevant information pertaining to clients’ charts.
- Maintaining fax/referral log
- Ensure compliance with the Health Insurance Portability and Accountability Act (HIPAA) confidentiality of all medical records at all times.
- Adherence to QCHC House Rules
- Other duties as assigned.
Qualifications
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of filing, coding, and collation of file
- Ability to maintain records and files.
- Ability to work in a fast-paced environment.
- Ability to maintain confidentiality.
- Strong organizational and communication skills.
QUALIFICATIONS:
Education:
- High school diploma or equivalent
- Associate degree in health information technologies and/or certification as a Registered Health Information Technician (RHIT)
Experience:
One (1) – Three (3) years of medical records experience preferred.
