Role Description This is a full-time Physical Therapist role based in Concord, NH, with a hybrid work arrangement that combines in-person patient visits with some work-from-home flexibility for documentation and care coordination. The Physical Therapist will assess patients in their homes or community settings, develop individualized treatment plans, and provide evidence-based interventions to improve mobility, strength, balance, and functional independence. Daily responsibilities include conducting comprehensive evaluations, setting measurable goals, delivering therapeutic exercises and manual therapy, and educating patients and caregivers on home exercise programs and safety strategies. The role also involves collaborating closely with an interdisciplinary team, communicating changes in patient status, and contributing to holistic care plans. The Physical Therapist will maintain accurate, timely documentation, participate in case conferences, and uphold regulatory and organizational standards for quality and patient-centered care.

Qualifications

  • Strong foundation in Medicine and clinical decision-making relevant to home health and hospice settings.
  • Demonstrated expertise in Physical Therapy, including evaluation, treatment planning, and evidence-based interventions.
  • Experience in Rehabilitation focused on improving mobility, function, and quality of life for diverse adult and geriatric populations.
  • Commitment to high-quality Patient Care, including education, safety, and clear communication with patients and families.
  • Knowledge of Home Care practices, including in-home assessments, infection control, and community-based resources.
  • Current state licensure as a Physical Therapist (or eligibility) in New Hampshire; graduation from an accredited PT program.
  • Valid driver’s license, reliable transportation, and ability to travel to patient homes within the service area.
  • Strong documentation skills, familiarity with electronic health records, and capacity to manage a dynamic caseload.
  • Excellent interpersonal skills, cultural sensitivity, and the ability to work collaboratively within an interdisciplinary team.
  • Prior home health or hospice experience is preferred but not required; willingness to learn and adapt to evolving patient needs.
