Position: Community Based Worker-

Quality Behavioral Health is looking for an outgoing person to assist its Community- Based Care Coordination Program in Asotin County. This candidate must have strong communication skills, the ability to be out in the community and build strong community partners.

Goal of Program:

To connect community members with local services and support what they need to be healthy using the training and deployment of Community Based Workers to build and strengthen community resilience by addressing health disparities, identifying Health-Related Social Needs and improving access to a medical home, and Health-Related Social Needs service providers.

Selected candidate will be trained to be a Community Based Worker, follow any certification requirements from the Department of Health. Individual who has a unique understanding of the experience, language, and culture of a particular population and provides culturally appropriate care coordination and resource navigation support to members of their community.

Responsibilities may include:

  • Securing or coordinating for the client, mental health, residential, social, vocational, health, educational, income management, and transportation.
  • Work as needed with clients' families, significant others, and community caregivers to help maintain the clients in the community and to be supportive and act in the best interest of the client.
  • Request, as needed, the consultation of specialists to provide specific assistance in working with any specific population requiring specialized knowledge.
