At Qualicare Home Care Princeton, we provide compassionate, personalized care that helps seniors thrive safely at home. We’re seeking a people-focused, driven Community Outreach & Partnerships Manager to expand our referral network and help more families access the care they deserve.
About the Role
This is a field-based, relationship-driven role where you’ll develop referral partnerships and manage the entire client acquisition process—from first contact to signed onboarding. You’ll represent Qualicare in the community, build trust with partners, and make a meaningful impact on the lives of seniors.
Key Responsibilities
- Build referral relationships with:
- Assisted Living & Skilled Nursing Facilities
- Rehab Centers, Hospice Providers, Hospitals
- Social Workers, Case Managers, Elder Law Attorneys
- Senior Centers, Faith-Based Orgs, Community Leaders
- Attend healthcare events, expos, and networking functions
- Manage the whole client journey from referral to onboarding
- Follow a proven outreach process (training provided)
- Track activity and results using CRM tools
- Collaborate with care teams to ensure seamless onboarding
- Offer ideas to improve outreach and partnership strategy
Who You Are
- Preferred experience:
- Social work, community relations, outreach, or senior care
- Military veterans with relevant leadership or service background encouraged to apply
- A strong communicator and natural relationship-builder
- High energy, quick thinker, self-motivated
- Comfortable following systems, and open to continuous improvement
- Must have a valid driver’s license and reliable transportation
Experience in elder care or healthcare is helpful—but not required. If you're passionate, coachable, and driven to serve, we want to hear from you.
Compensation & Growth
- Base salary: $45,000–$55,000
- Commission plan with earning potential of $90,000
