The Cedars Skilled Nursing is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our rehabilitation team on a per diem basis.
The SLP will provide skilled speech, language, cognitive, and swallowing services to residents in our skilled nursing and subacute rehabilitation setting. This is an excellent opportunity for a clinician who enjoys working directly with patients and collaborating with an interdisciplinary team to help residents achieve their highest level of function and quality of life.
Responsibilities
- Conduct comprehensive evaluations of speech, language, communication, cognition, and swallowing as clinically indicated.
- Develop individualized treatment plans based on residents' clinical needs and functional goals.
- Provide skilled speech-language therapy interventions according to established plans of care.
- Evaluate and treat swallowing and feeding disorders and provide appropriate recommendations for safe eating and drinking.
- Provide cognitive-communication therapy and interventions as appropriate.
- Assist residents in improving communication, cognition, and functional independence.
- Provide education and training to residents, families, caregivers, nursing staff, and other members of the healthcare team.
- Monitor resident progress and modify treatment approaches as appropriate.
- Communicate changes in resident status and treatment recommendations to the interdisciplinary team.
- Maintain accurate, timely, and complete clinical documentation.
- Participate in care planning and discharge planning as appropriate.
- Promote resident safety, dignity, independence, and quality of life.
- Follow all applicable professional standards, facility policies, and state and federal regulations.
Qualifications
- Current Speech-Language Pathologist license in the State of Rhode Island or eligibility for Rhode Island licensure.
- Master's degree in Speech-Language Pathology from an accredited program.
- Certificate of Clinical Competence (CCC-SLP) through ASHA, or appropriate status/eligibility.
- Current CPR/BLS certification, or ability to obtain as required.
- Strong clinical, communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
- Experience in skilled nursing, subacute rehabilitation, long-term care, or post-acute care is preferred but not required.
