- Address: 6226 University Park Drive, Radford, VA, 24141
- Hourly Rate: $22.00/hour - $28.00/hour (based on education and experience)
- Schedule: Monday - Friday 9am-5pm
- Review clinical documentation, progress notes and treatment plans daily to ensure completeness, accuracy and alignment with behavioral health and substance use treatment requirements.
- Identify missing, incomplete or unclear documentation and communicate directly with clinicians to request corrections or additional information.
- Monitor completion of requested corrections and maintain accurate tracking of outstanding documentation needs.
- Escalate unresolved, repeated or serious documentation concerns to the Clinical Director or Clinical Supervisor only when necessary.
- Ensure documentation supports medical necessity, payer expectations and audit readiness by verifying required elements, signatures, dates and treatment alignment.
- Maintain organized documentation logs and partner with clinical leadership to address recurring gaps or compliance risks.
- Protect confidentiality at all times and adhere to all HIPAA, state and organizational standards for behavioral health and substance use records.
- Must exercise discretion and maintain confidentiality with regard to all company information.
- Completion of all required trainings as designated by the company and accreditation/licensing entities.
- Other duties as assigned
- High School Diploma or GED required; Bachelor’s Degree in a human services–related field preferred (psychology, social work, counseling, health information, behavioral health or similar).
- Minimum two years of experience reviewing clinical documentation in a behavioral health, substance use, quality assurance, utilization review or medical records setting, including review of treatment plans and progress notes.
- Experience using an EHR required; strong understanding of behavioral health terminology, treatment plan structure, payer documentation expectations and medical necessity standards preferred.
- Clinical licensure (CSAC, QMHP, LCSW, LPC, LSATP) or candidates under board-approved supervision strongly desired
