Schedule: Monday – Friday | 8:00 AM – 4:00 PM
Pay Rate: $16/hour

We are excited to announce an opening for a Case Coordinator who will be instrumental in coordinating aftercare appointments, maintaining communication with referral sources, and keeping legal or external agencies informed of our clients' progress.

Key Responsibilities
- Ensure clients have access to the full continuum of care by assessing their needs and facilitating appropriate services.
- Coordinate all appointments, aftercare plans, schedules, and meetings.
- Arrange Halfway House interviews and placements.
- Obtain necessary information and releases from clients, families, referral sources, and other agencies.
- Perform other duties as assigned.
