Role Description The Case Manager is a full-time, on-site role based in Springfield, MA. This role is responsible for coordinating care and services for clients receiving mental health and substance use treatment, ensuring that individualized treatment plans are implemented and updated as needed. Daily responsibilities include conducting intake assessments, gathering psychosocial histories, and collaborating with clinical and medical staff to develop person-centered care plans. The Case Manager will provide ongoing case management, including linking clients to community resources, coordinating appointments, and monitoring progress toward treatment goals. Additional tasks include maintaining accurate and timely documentation, communicating with families and external providers (with appropriate consent), supporting discharge planning, and participating in interdisciplinary team meetings to promote continuity of care. The role also involves advocating for client needs, supporting crisis intervention efforts, and modeling a recovery-oriented, trauma-informed approach.
Qualifications
- Strong case management and care coordination skills, including assessment, treatment planning, and discharge planning.
- Ability to build therapeutic relationships, demonstrate empathy, and communicate effectively with clients, families, and multidisciplinary teams.
- Solid organizational and time-management skills, with the ability to manage a caseload, prioritize tasks, and meet documentation deadlines.
- Knowledge of behavioral health, substance use disorders, recovery supports, and community resources.
