Physical Therapist (PT) | Outpatient Rehabilitation
 

We are seeking a compassionate and motivated Physical Therapist (PT) to join our outpatient rehabilitation team. The ideal candidate will be responsible for evaluating, developing, and implementing individualized treatment plans to help patients improve mobility, reduce pain, restore function, and achieve their rehabilitation goals.

• Perform comprehensive patient evaluations and assessments
• Develop and implement individualized treatment plans based on patient needs and goals
• Provide therapeutic interventions to improve mobility, strength, balance, and function
• Monitor patient progress and adjust treatment plans as appropriate
• Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention
• Maintain accurate and timely documentation in accordance with facility and regulatory standards
• Collaborate with physicians and interdisciplinary healthcare teams to ensure optimal patient outcomes

• Active Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a multidisciplinary team
• Outpatient and orthopedic experience preferred

• Experience treating orthopedic and musculoskeletal conditions
• Knowledge of evidence-based rehabilitation practices
• Strong patient education and care coordination skills
• Ability to manage a diverse caseload in a fast-paced environment
