Key Responsibilities:
- Direct Patient Care: Room patients, perform comprehensive physical assessments, obtain vital signs, administer medications and vaccines, perform phlebotomy and EKGs, and conduct 6-minute walk tests for heart failure patients.
- Chronic Disease Management: Provide extensive education on heart failure self-management, medication adherence, dietary restrictions (e.g., low sodium), symptom monitoring, and LVAD care (if applicable).
- Care Coordination & Triage: Manage telephone and Epic in-basket triage, collaborate closely with cardiologists, nurse navigators, social workers, and palliative care specialists to facilitate seamless patient care and resource connection.
- Palliative Care Support: Assist in providing holistic, patient-centered palliative care, focusing on symptom management, quality of life, and aligning medical care with patient goals within the heart failure population.
- Team Leadership: Delegate and supervise care provided by non-licensed clinical staff within the clinic.
- Clinic Operations: Thrive in a fast-paced outpatient environment focused on improving clinical outcomes and preventing hospital readmissions for patients with advanced cardiac conditions.
- Skills-Current RN license (Georgia or Compact State)
- Current BLS certification (required upon hire)
- Strong clinical assessment and patient education skills
- Interest or experience in cardiology, heart failure, palliative care, or chronic disease management
- Diploma in Nursing or Bachelor of Science in Nursing (BSN) is required.
- BSN is strongly preferred.
- Current RN License in the State of Georgia or a Multi-State Compact License.
- Current BLS (Basic Life Support) certification required upon hire.
- ACLS certification is highly desirable.
- Certification in Heart Failure (CHFN) or Palliative Care (CHPN) is a significant plus.
- Experience with direct patient care is preferred.
- Experience in Cardiology, Heart Failure, Telemetry, ICU, Palliative Care, or Chronic Disease Management is highly desirable. New graduates with relevant clinical rotations are encouraged to apply.
- Strong clinical assessment, critical thinking, and patient education abilities.
- Excellent interpersonal, teamwork, and communication skills.
- Ability to multitask, triage effectively, and work collaboratively within an interdisciplinary team.
- Proficiency with electronic health records (Epic) and clinic technology.
