Pay Range: Regular Pay: $65.00/hr.

Key Responsibilities:

• Assess patient/client needs and develop/implement care plans
• Coordinate services and advocate for clients
• Monitor progress to ensure clients receive necessary support and resources
• Evaluate patients in ED for medical necessity and care coordination needs
• Propose and implement outpatient plans for safe discharge
• Coordinate care for patients with abuse, chronic pain, and frequent utilization issues
• Interact with individuals with mental health issues to help them access resources
• Work closely with Providers, Nurses, Case Managers, Physicians, PRC department, and all healthcare providers
• Collaborate with interdisciplinary team to assess care plans and make recommendations
• Review patient cases in inpatient and outpatient services departments
• $65/hr on W2! – great pay for experienced Clinical Social Workers!

• Contract Length: 13 weeks
• Shift: Days – Monday–Friday, 8:00 AM – 5:00 PM – Callback on weekends/holidays required
• Expected Hours: No hourly guarantee

Key Requirements and Technology Experience:

• Education: Master's degree in Social Work (MSW) required
• License/Certification:
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required
• Valid, current, full, and unrestricted Professional License from any US state, Puerto Rico, or US territory
• Current BLS certification required (AHA)
• Experience:
• Minimum 3 years of experience in a clinical care setting
• Experience with discharge planning, case management, or utilization review processes
• Demonstrated knowledge of electronic health record systems (Allscripts EMR preferred)
• Holiday coverage required – callback on weekends/holidays
• Housing Note: Hospital rental property very limited. Shared contractor housing only. No pets or family allowed per hospital policy.
• Housing application must be submitted 2 weeks prior to start.
• Orientation: Every 2 weeks
• COVID & Flu Exemptions: May accept after declination form is submitted
• Locals are accepted – no radius restriction
